How to Setup Custom Contact Form Plugin in WP

Step 1:

Download the plugin here, check this out in case you need the guide on how to install plugin in wp.

Step 2:

Under “Custom Contact Forms” tab on your left side bar, click on the “Custom Contact Forms” link.

Step 3:

Once clicked, you will be directed to the plugin page, click on the “forms” tab and fill up the field as below:

Form slug: Key in a unique name for this form.

Form title: If you wish the form title to be displayed when visitors fill up the form, key in a title here. Example – Contact us, advertise with us and etc.

Submit button text: This is the text displayed on the form submission button. Key in “submit”, “send” and etc.

Form destination email: Key in the email address in which you need the form to be submitted to.

Form email subject: Key in a subject if you wish it to appear when the form sent to your email.

Form email name: Key in a name if you wish it to appear when the form sent to your email.

Form success message: When this message is filled, a message will be displayed when the form is successfully filled up.

Form success message title: When this title is filled, a message with the message title will be displayed when the form is successfully filled up.

Step 4:

Leave all other settings as it is and click on “Create form”.

Step 5:

Click on the “fields” tab, this allows you to create a list of information that you want the visitors to fill in. Follow the recommended setting below to create the fields of your form.

Field slug: Key in a unique name for this field.

Field label: The field label is displayed next to the field.

Field type: Depending on what type of information you wish to collect, choose the appropriate option from the drop down list.

Required field: If “Yes”, means the field must be filled up by the visitors.

Step 6:

Leave all other settings as it is and click on “Create field”. Create as many field as you like.

Step 7:

Once the form and fields are created, click on the “Forms” tab again. Scroll down to the “Manage forms” section, click on the “options” button from the created form.

Step 8:

Once clicked, the form will be expanded. Under the “Add a field” section, choose the field that you have just created and click “Attach”. After all required fields are attached, click on the “Save field configuration” button. There is a very important field created on default called “captcha”, always attach this field at the last section of your form, this is to prevent bot spammer to fill up your form.

Step 9:

Once all fields attached and click on the “save” button.

Step 10:

Once saved, copy the “Form display code” (example: [customcontact form=1]) and paste into wherever page/post that you wish to display the form.
